Create custom forms and use them from the Electronic health record page.


How to Set or Update Forms?


  • Click on the settings button available on the left side menu or click the profile picture and choose it from the dropdown. Navigate to the Forms section and click on the Set Forms button.
  • Click on the add button to Create new custom forms, add the required details, and click on the Save button.
  • Click on an already created form, make the required changes, and click on the Save button
